OPT Health Insurance Plan

Please note, as of the Fall 2024 semester, ALL SUNY Oswego OPT and STEM OPT Participants are expected to remain covered under the SUNY Oswego International Student Health Insurance plan. Waivers will only be available for OPT or STEM OPT Participants who are EMPLOYED AND can provide proof of health insurance from their Employer.

I confirm that I have read and agree to the above statement.

ALL SUNY Oswego OPT and STEM OPT Participants will be billed on their SUNY Oswego Student Bill for the International Student Health Insurance plan premiums. ALL OPT and STEM Participants are required to pay this bill. Payment plan options can be discussed with the SUNY Oswego Student Accounts Department.

I confirm that I have read and agree to comply with the above.

I understand that my Health Insurance payment can be reimbursed on a prorated scale if I provide the ISSS Office with proof of employment and health insurance from my employer. The letter must demonstrate the START DATE of your Employer's insurance and must be sent to the ISSS Office via email at isss@oswego.edu. The premium reimbursement will be based on the date the proof of coverage is submitted to the ISSS office. Please note, NO previous semester adjustments can be made. Adjustments can only be processed in the current semester.

International Student OPT Acknowledgement

All international students who intend to participate in Optional Practical Training (OPT) must abide by all rules associated with this practical training experience in order to maintain their F-1 student visa status.

Please read each of the statements listed below carefully.

I understand that I must complete all of my degree requirements by my intended graduation date in order to apply for OPT and that it is my responsibility to ensure that I graduate on the program end date indicated on my OPT I-20.

I understand that participation in OPT is not guaranteed. Therefore, I will take the appropriate steps to make sure that I have a plan for my employment in the event that I need to return to my home country after graduation.

I understand that an OPT application is my own personal application and that I alone am responsible for timely filing with complete documentation, understanding F-1 rules regarding OPT, and properly maintaining my F-1 visa status.

I acknowledge that it generally takes between 3 - 5 months after the date on which my OPT application was filed in order to receive my EAD card, and that I can check updates on my case through my USCIS account or by downloading the "Case Tracker for USCIS" app

I understand that, if I would like my EAD Card mailed to the SUNY Oswego campus, I should use the ISSS Office address of 301 Washington Blvd, 116 Sheldon Hall, Oswego NY 13126.
